Job Description:
Duties include, but are not limited to: -Evaluate and assign
patient referrals -Ensure continual delivery of high quality care
-Collaborate with physicians and community resources -Evaluate the
performance of staff -Maintain reports -Prepare schedules -Hold
monthly inservice programs -Maintain QAPI and Emergency preparation
programs -Writing and updating policies and procedures Skills and
specifications: -Excellent communication skills -Home health
experience -Excellent clinical skills -Knowledge of Electronic
Medical Records -Excellent time management skills and ability to
work under pressure -Knowledge of Department of Health regulations
and Federal Conditions of Participation -Knowledge of OASIS
Education and Qualification: Valid PA Nursing license Valid
Driver's license 5+ years nursing experience Ref: OS
