Job SummaryFacilitates processes on a daily basis to expedite
patient's care across the continuum. Coordinates interdisciplinary
care services. Interviews each new patient and/or family on
admission and formulates expected outcomes and discharge plan
assuring continuity of care. Facilitates the patient and family
toward expected outcomes. Insures appropriate use of hospital
resources by reviewing medical records of all patients admitted to
the hospital and evaluating appropriateness of admission and
continued stay.
Job Duties
- Using utilization management principles, insures that patient's
care is necessary and appropriate, covered by third party payors
and follow up is complete.
- Screen for quality eventsmanual for morbidity and
mortality.
- Enters focus study for blood and blood products.
- Performs quality entry for related admissions.
- Uses case management skills insuring that patients move
efficiently along the continuum of care.
- Reviews the medical record and screens each new patient
admission to the unit discussing the expected length of stay and
anticipate post hospital nursing care needs Works cooperatively as
a member of the interdisciplinary team to insure appropriate use of
hospital resources and progression of care.
- Collaborates with the interdisciplinary team to develop, review
and revised the patient's point of care.
- Assists in obtaining vital results from diagnostic testing and
assures they are communicated to the physician.
- Investigates and attempts to resolve delays, duplication or
fragmentation of patient care service to improve efficiency and
quality of care.
- Reports these issues to the appropriate department.
- Refers to social services any case requiring consultation due
to, but not limited to, family dynamics, need for financial
assistance, housing, maternal and child concerns and community
service including home health services or placement for lesser
level of care.
- Identifies patients with suspected infections and refers to the
Infection Control Coordinator.
- Encourages outpatient testing when appropriate.Work is
typically performed in a clinical environment. Accountable for
satisfying all job specific obligations and complying with all

- Hours: 11AM to 9:30PM
- Weekends/Holidays: rotation of weekends and holidays - will be
reviewed at interview
