Medna Inc. is seeking Contract Nurse Evaluators to perform UAS-NY (Uniform Assessment System for New York) assessments for patients enrolled in the NYS NHTD and TBI Waiver programs. This role offers flexible hours based on your availability, making it an excellent opportunity for nurses looking for a dynamic and rewarding career.Key Responsibilities:

• Conduct in-home UAS-NY assessments for patients across NYC boroughs (Bronx, Brooklyn, Queens, Manhattan, Staten Island).
• Accurately document and report assessment findings in compliance with NYS guidelines.
• Collaborate with the Medna team to create individualized care plans based on assessment results.
• Ensure compliance with all NYS regulations and standards for UAS-NY assessments.
• Travel to patient homes for approximately 50% of the role; the remaining work is remote.
• Participate in training sessions to become proficient in UAS-NY (if no prior experience).Qualifications:
  • Education: Active RN license in the state of New York (required).
  • Experience: Familiarity with UAS-NY is preferred but not required (training will be provided).
  • Skills: Strong clinical assessment skills, excellent communication, and attention to detail.
  • Travel: Willingness to travel to patient homes within NYC boroughs. Reliable transportation is required.
