Job Description:
Salary: $38.00-$53.20 Overview:The Transport RN is responsible
for planning, organizing and assisting with transport of assigned
individuals to off-site appointments. This individual ensures the
general health and well-being of individuals by providing direct
nursing care throughout transport. Direct nursing care of residents
encompasses planning, developing, organizing, implementing
assessing, and evaluating the care of individuals to ensure that
the highest degree of quality care and regulatory compliance is
maintained at all times. Within the RNs scope of practice, this
individual also monitors performance of non-licensed personnel and
assists in modifying treatment regimens to meet the physical,
psychosocial and developmental needs of the individual in
accordance with established nursing practices and the requirements
of the policies and procedures of the organization in accordance
with current federal, state, and local standards governing the
facility.Responsibilities:Day-to-Day tasks include, but are not
limited to:Assist in the transport of our children to their
appointments outside the facilityScreen and provide an in-depth
evaluation of the needs of ventilator & respiratory clientsConsult
with the attending physician regarding treatment protocol and make
written recommendations for interdisciplinary care planning and
treatmentInform residents or qualified representative about care
and treatment plan and possible outcomes of treatment, or lack of
treatment if resident refuses participation in a respiratory
rehabilitation programIdentify problems and develop a complete plan
of care according to facility/unit policy and federal/state
regulationsInitiate ventilator weaning protocol and/or respiratory
rehabilitation in accordance with the physicians orders; maintain
required contact with physicianMaintain written records,
evaluations, progress notes, and discharge summaries of residents
receiving respiratory treatment and monitoring according to state,
federal, and facility policiesAttend, as necessary, and provide
input at the Resident Care Planning (RCP) meetings for
interdisciplinary team and the Resident/Family Care Plan Review
Meeting; complete required documentation after the meetingsRecord
residents plan of care in the Interdisciplinary Care Plan as
required or as neededContribute to the completion of the Minimum
Data Set (MDS) within the established time frame and incorporate
the results in the plan of care when appropriateConfer with
personnel of other departments and provide required training to
meet special needs of residentsQualifications:Valid PA State
license/registration to practice as Registered Licensed Nurse
(RN)Minimum of two (2) years (RN) nursing experienceAssociates of
Nursing required; Baccalaureate Degree in Nursing (BSN)
preferredPediatric experience strongly preferredAbility to
supervise and work well with other personnel requiredQualities of
maturity, diplomacy, and ability to work well with others
requiredCaring, respectful attitude towards residents requiredThis
position requires regular interaction with residents, coworkers,
visitors, and/or supervisors. In order to ensure a safe work
environment for residents, coworkers, visitors, and/or supervisors
of the Company, and to permit unfettered communication between the
employee and those residents, coworkers, visitors, and supervisors,
this position requires that the employee be able to read, write,
speak, and understand the English language at an intermediate or
