With Providers On Demand, we ensure that patients receive the care they need, when and where they need it most.Reports To: Clinical Advanced Practice Provider DirectorJob Summary:The Psychiatric Mental Health Nurse Practitioner (PMHNP) plays a critical role in delivering high-quality healthcare services, primarily through telehealth platforms. In this role, you will provide virtual mental health care for Medicare-eligible patients enrolled in a Chronic Care Management (CCM) program, focusing on individuals with multiple chronic medical conditions, behavioral health needs, and complex care plans. The PMHNP is responsible for psychiatric evaluations, medication management, supportive therapy, and active participation in interdisciplinary care coordination to promote patient well-being and improve health outcomes.This role emphasizes integrated care delivery where behavioral health is addressed alongside chronic medical conditions like diabetes, heart failure, COPD, hypertension, and cognitive decline.
