Summary:The Psychiatric Mental Health Nurse Practitioner (PMHNP)
plays a critical role in delivering high-quality healthcare
services, primarily through telehealth platforms. In this role, you
will provide virtual mental health care for Medicare-eligible
patients enrolled in a Chronic Care Management (CCM) program,
focusing on individuals with multiple chronic medical conditions,
behavioral health needs, and complex care plans. The PMHNP is
responsible for psychiatric evaluations, medication management,
supportive therapy, and active participation in interdisciplinary
care coordination to promote patient well-being and improve health
outcomes.This role emphasizes integrated care delivery where
behavioral health is addressed alongside chronic medical conditions
like diabetes, heart failure, COPD, hypertension, and cognitive
decline. This role emphasizes cultural sensitivity, adherence to
state-specific telehealth regulations, and collaboration with
interdisciplinary teams to enhance patient outcomes and bridge

communities, and advancing your career.Key ResponsibilitiesConduct
telehealth-based psychiatric evaluations and behavioral health
screenings (PHQ-9, GAD-7, MoCA, etc.).Diagnose and manage mental
health conditions in medically complex populations.Prescribe and
monitor psychotropic medications, considering interactions with
chronic disease treatments.Collaborate with clinical care teams to
integrate behavioral health into CCM plans.Participate in team
meetings and case reviews to support coordinated, whole-person
care.Ensure documentation meets Medicare, HIPAA, and NYS telehealth
compliance standards.Develop crisis plans when needed and
coordinate emergency support if risks are
identified.RequirementsMasters or Doctorate in Nursing (MSN or
DNP).Board certification as a PMHNP (ANCC).Active RN and NP license
in New York State with full practice authority.Current DEA license
with authority to prescribe Schedule II-V in NY.Minimum 2 years of
PMHNP experience (telehealth or home-based care a plus).Strong
cultural competence and communication skills.Experience working
with underserved or older adult populations
preferred.Technologically proficient and comfortable using EHRs and
telemedicine tools.Mandarin or Spanish language skills highly
