Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.Job DescriptionPURPOSE OF POSITIONThe Wound Specialist will be responsible for providing guidance and recommendations to a clinical team in the coordination and rendering of specialized wound and ostomy care delivery in the Certified Home Health Agency. This team will include Registered Nurse but may extend to other members of the interdisciplinary care team. The Wound Specialist is a part of the interdisciplinary team that will focus on ensuring the assigned clinicians strive to achieve the quality patient care outcomes that are steeped in evidence-based care and practices. In addition, Wound Specialist will engage in efficient advisement, coaching and mentoring strategies to the members of the care team in order to meet clinical and professional goals. The Wound Specialist will stay abreast of wound and ostomy care industry trends and regulatory guidelines that govern the home health care team and clinical practice.Minimum QualificationsQualification Requirements:

  • Valid license and current registration to practice as a Registered Professional Nurse issued by the New York State Education Department (NYSED); and
  • Current certification in one of the following:
  • Certified Wound Ostomy Continence Nurse (CWOCN) issued by the Wound, Ostomy and Continence Nursing Certification Board (WOCNCB); or
  • Certified Wound Ostomy Nurse (CWON) issued by the WOCNCB; or
  • Certified Wound Care Nurse (CWCN) issued by the (WOCNCB); or
  • Certified Wound Specialist (CWS) issued by the American Board of Wound Management (ABWM); or
  • Wound Care Certified Certification (WCC) issued by the National Alliance of Wound Care and Ostomy (NAWCO).
