Surg for a local contract nursing job in Brooklyn, New York. Job

Duration: 13 weeks 40 hours per week Shift: 8 hours, days
Employment Type: Local Contract A large, well-known social services
agency is seeking a full-time Registered Nurse for a position in
Brooklyn, NY. This is a temp-to-perm opportunity! Job Function: Day
Shift 9am - 5pm / 8am - 4pm. Conducts timely client assessments,
completing initial evaluations within five visits or thirty days of
registration, and reassessing as needed on an ongoing basis.
Coordinates comprehensive care plans by collaborating with
interdisciplinary team members and integrating PCP or clinic
treatment orders when appropriate. Manages a client caseload to
ensure consistent care across the ADHC and with external medical
and case management providers. Provides client education and
follow-up, including HIV/AIDS prevention and risk reduction
strategies, documenting all medical visits and ensuring
communication with primary care providers. Job Requirements:
Registered Nurse licensed by the State of New York. Bachelor’s
degree from an accredited college or university. 1–2 years of
experience in mental health. Bilingual in Spanish (preferred).
