experienced Hospice Case Manager Registered Nurse (RN) for a
contract in service areas Sussex and Waren Counties, NJ and Pike
County, PA! This assignment is for 13 weeks and ideal candidates
will have at least six months of recent experience as a Case
Manager and Home Health RN. Hospice Case Manager Registered Nurse
(RN) Position Details: • Contract Length: 13 weeks • Location:
Sussex and Waren Counties, NJ and Pike County, PA • Facility Type:
Hospice Facilities and Home Based • Shift & Schedule: Full-Time, 8
Hour Days, Monday to Friday, On-call Required Hospice Case Manager
Registered Nurse (RN) Requirements: • Must have six months of
recent Case Manager experience in a Hospice facility and Home Based
settings • Active New Jersey, Pennsylvania, or Compact State
Registered Nurse License required • Current BLS Certification
