Job Description Job Description Role and Responsibilities
Direct, supervise, evaluate and provide speech therapy services to
patients in the home, as prescribed by the attending physician.
Essential Functions Evaluate the patient’s speech, language and
voice, and swallowing abilities according to referral and perform
periodic reevaluation. Develop and implement rehabilitative
services for speech, language and voice, and swallowing disorders.
Select and administer diagnostic tests and provide appropriate
therapeutic techniques and materials. Record the type of treatment
and patient’s reaction to it on the clinical/progress notes.
Maintain adequate records on all of the patients, including summary
reports. Instruct and educate family members and patient, regarding
treatment plan and therapy carry-over. Attend case conferences.
Participate in the development of and periodic revision of the
physician’s plan of care. Other duties as assigned. This job

any time. Education Requirements Bachelor’s degree and Master’s
degree in Speech Pathology. Must have "Certificate of Clinical
Competency" from an accredited institution. Qualifications and
Requirements Current, valid license issued by NYS Department of
Education. Work Environment The Speech Language Pathologist is
primarily a non-office setting and may be exposed to outdoor
conditions. The working conditions are classified as light work:
Light work - Exerting up to 20 pounds of force occasionally, and/or
a negligible amount of force frequently or constantly to lift,
carry, push, pull or otherwise move objects, including the human
body. Light work involves sitting most of the time. Physical
