eligibility, Medicaid issues, and keeping our clients connected

advancement. Key Responsibilities: Patient Retention: Manage a full
schedule of calls with existing clients to ensure they continue
receiving our services. Medicaid & Medicals: Follow up on Medicaid
eligibility issues and obtain updated medical documentation from
patients. Process Management: Stay organized and ensure all
processes are followed to retain patient services. Collaboration:
Transfer qualified and converted callers to the appropriate
departments and manage patient information in Salesforce. Patient
Relations: Work with diverse patients across New York State,
providing top-notch customer service. What Were Looking For:
Education: High school diploma or equivalent (required). Language
Skills: Bilingual in Spanish (preferred). Tech-Savvy: Comfort with
Microsoft Office, especially Excel. Communication: Strong verbal
skills, including listening, advising, and consulting. Organized &
Driven: Ability to manage time effectively and work independently
or as part of a team. Experience: Prior experience in home health
care and knowledge of HHA Exchange (a plus). What We Offer:
