Job Description:
Job Description Job Description Job Summary: We are currently
seeking Board Certified or Board Eligible Physicians to join our
expanding primary care team in the Nursing Home Division. This role
involves providing high-quality treatment and diagnostic services
to patients in various nursing home settings across New York. Key
Responsibilities: Deliver primary medical care services within the
standard of care for nursing home patients. Order and interpret
laboratory and radiology tests; refer patients to specialists when
necessary. Analyze medical records, test results, and examinations
to diagnose conditions and propose treatment plans. Prescribe
medications and treatment regimens for identified medical
conditions. Educate patients on potential side effects of
treatments and ensure allergy information is current. Advise
patients on lifestyle and dietary changes to improve health
outcomes. Adhere to departmental policies, quality improvement
initiatives, and safety standards. Maintain patient confidentiality
and comply with federal and state health information privacy laws.
Document complete, timely, and legible medical records.
Qualifications: Board Certified or Board Eligible in Geriatrics,
Internal Medicine, Palliative Care/Hospice, or Family Medicine. MD
or DO degree from an accredited U.S. medical school. Completion of
residency at an accredited healthcare institution or through a U.S.
residency or 5-year pathway. Active New York State Medical License.
DEA and DPS registration for prescription writing (can be obtained
during credentialing). Experience in providing primary care and
diagnostic services in a clinical setting. Ability to work
collaboratively within a multidisciplinary healthcare team. Company
