Job Description Job Description Wound Care Nurse (RN) Brooklyn,
NY Full-Time | Skilled Nursing / Long-Term Care We are seeking a
clinically strong Wound Care Nurse to lead skin integrity
initiatives within a high-acuity Skilled Nursing facility in
Brooklyn. This is a highly visible role that directly impacts
resident outcomes, survey performance, and quality metrics. The
ideal candidate is hands-on, detail-oriented, and confident in both
advanced wound treatment and regulatory documentation. Position
Overview The Wound Care Nurse is responsible for comprehensive
wound assessment, treatment planning, documentation accuracy, and
facility-wide education. You will collaborate closely with
physicians, nursing leadership, rehab, dietary, and infection
control teams to ensure best-in-class wound management and
prevention. Key Responsibilities • Conduct comprehensive skin
assessments upon admission and routinely thereafter • Stage and
document pressure injuries accurately in accordance with NPIAP
guidelines • Develop and oversee individualized wound treatment
plans • Perform and supervise advanced wound care treatments and
specialty dressings • Lead weekly wound rounds with
interdisciplinary teams • Partner with MDS to ensure accurate
coding and compliance • Maintain survey readiness related to skin
integrity standards • Track wound metrics and participate in QAPI
initiatives • Educate nursing staff and CNAs on prevention
protocols and documentation standards Qualifications • Active,
unencumbered RN license in New York • 2 years of wound care
experience in Skilled Nursing or Sub-Acute setting • Strong
knowledge of pressure injury staging and prevention • Familiarity
with MDS 3.0 and Quality Measures • Experience with EHR systems
(PointClickCare or SigmaCare preferred) • Wound Care Certification
(WCC, CWCN, or similar) preferred Why This Role Stands Out •
