Job Description:
A healthcare provider serving assisted living residents is
seeking a Medicaid-credentialed Nurse Practitioner for an
office-based role providing virtual patient care. The position
offers stable daytime hours and requires strong clinical judgment
and documentation skills. Responsibilities Include: Conducting
virtual patient assessments and developing treatment plans
Providing remote clinical services to assisted living residents
Documenting all care accurately and thoroughly Collaborating with
the clinical team to support patient outcomes Ideal Qualifications:
Must be Medicaid-credentialed Strong clinical assessment and
communication skills Detail-oriented and comfortable with virtual
care platforms Prior geriatric or assisted living experience is a
