Summary of Responsibilities:
Under the general direction of the Unit Supervisor, thisperson is
responsible for providing support coordination services
toindividuals assigned. Using helping skills and a strengths-based
approach, thisperson builds a trusting rapport with individuals to
better understand theirunique needs, interests, and their vision of
a good life. This person providesongoing monitoring, support, and
linkage to available systems and supportswithin the community.
Supports Coordinators promote the philosophies of EverydayLives,
Values in Action, and Self Determination withinservice
Lead teams to support the people we serve;Collaborate with
individuals, families, and providers to ensure people areliving
their best lives; Facilitate meetings and discussions; Advocate;
Criticallythink and problem solve; Links individuals to community
resources, funding, andservices; Assess needs; Create plans;
Document service provided; Empowerindividuals in choice and the
right to lead an EverydayLife.

Education and Experience:
Bachelor's degree, which includes or is supplemented by atleast 12
college credits in sociology, social welfare, psychology,
gerontology,criminal justice, or other related social science, or
two years' experience asa County Social Service Aide 3 and two
years of college-level course work,which includes at least 12
college credits in sociology, social welfare,psychology,
gerontology, criminal justice, or other related social service,
orany equivalent combination of experience and training which
includes 12 collegecredits in sociology, social welfare,
psychology, gerontology, criminaljustice, or other related social
service and one year of experience as a CountySocial Services Aide
3 or similar position performing paraprofessional casemanagement
Automobile and Valid Insurance, ValidPennsylvania Driver's License,
acceptable Motor Vehicle Record Check, Act 33and Act 34 Clearances,
FBI Fingerprint Clearance, Social Security Card,Immigration and
Naturalization (I-9), Drug Screening Clearance.
Prolonged periods working on a computer in an open
officeenvironment, comfortable communicating through virtual
technology, and travelthroughout the assigned region may occur and
may be in adverse weatherconditions, must be able to lift up to 15
